PHP Medicare is an HMO-POS plan with a Medicare contract. Enrollment in a PHP Medicare plan depends on contract renewal. All PHP Medicare plans include Part D drug coverage.

To enroll in a PHP Medicare plan, you must have both Medicare Parts A and B and reside in the plan service area. You must continue to pay your Medicare Part B premium. Enrollment in a PHP Medicare plan may be limited to specific times of the year.

Out-of-network/non-contracted providers are under no obligation to treat PHP Medicare members, except in emergency situations. Please call our Customer Service number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.

Every year, Medicare evaluates plans based on a 5-star rating system.

PHP Medicare complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability or sex.
